Aplastische Anämie

Definition:
Bi- oder Panzytopenie und hypozelluläres Knochenmark ohne morphologische Anomalien der Stammzellen. Wahrscheinlich autoimmune Genese in der Mehrzahl der Fälle.
Häufigkeit:
Selten: In Mitteleuropa 2 bis 3 Fälle pro Million und Jahr.
Symptome:
Abhängig davon, welche Zellreihe am meisten betroffen ist: Dyspnoe, Müdigkeit, rezidivierende Infektionen.
Befunde:
Anämiezeichen wie Blässe,Tachykardie. Evtl. Zeichen einer erhöhten Blutungsneigung wie Petechien, Hämatome. Keine Hepatosplenomegalie, Lymphadenopathie oder Knochenschmerzen.
Diagnostik:
Hb, Differenzialblutbild, Hämolyseparameter, Gerinnung. Eine Knochenmarkbiopsie bestätigt die Diagnose.
Therapie:
Abhängig von Alter, Ursache und Schweregrad: supportive Transfusionen, immunsupressive Behandlung, evtl. Thrombopoetinagonist. Knochenmarktransplantation als langfristig wirksamste Option für ausgewählte Patient*innen. Bei niedrigem Schweregrad genügt oft eine regelmäßige Überwachung des Blutbilds ohne Behandlung.
