Polyzystisches Ovarsyndrom (PCOS)

Definition:
Das polyzystische Ovarsyndrom (PCOS) ist eine heterogene Erkrankung, die sich durch Anovulation und Infertilität, Zyklusstörungen und Hyperandrogenismus auszeichnet.
Häufigkeit:
Tritt bei ca. 5‒10 % aller Frauen im fertilen Alter auf. Schleichender Beginn im Alter zwischen 15 und 25 Jahren.
Symptome:
Menstruationsstörungen, Infertilität, Hirsutismus, Akne, Übergewicht und metabolisches Syndrom.
Befunde:
Zyklusstörungen, evtl. Hirsutismus, Übergewicht, Virilisierungserscheinungen.
Diagnostik:
FSH normal, LH erhöht, Prolaktin gelegentlich erhöht, Testosteron leicht erhöht, häufig hohes LDL und niedriges HDL, mit einem LH/FSH-Quotient > 2.
Therapie:
Gewichtsreduktion, Ovulationsstimulation (bei Kinderwunsch) und Behandlung des Hyperandrogenismus durch Endokrinolog*in.
