Morbus Menière

Definition:
Erkrankung des Innenohrs aufgrund endolymphatischen Hydrops.
Häufigkeit:
In Deutschland pro Jahr geschätzt 3.200–9.000 Neuerkrankungen.
Symptome:
Akute Attacken mit Drehschwindel von 20 min bis 12 h. Zusätzlich fluktuierende Hörstörung, Tinnitus und Druckgefühl im Ohr, meist unilateral.
Befunde:
Während Attacke Nystagmus und Hörstörung.
Diagnostik:
Nachweis des asymmetrischen Hörverlusts im Tonaudiogramm. MRT zum Ausschluss intrakranieller Neoplasien oder entzündlicher Veränderungen.
Therapie:
Akuttherapie mit Antiemetika. Anfallsprophylaxe mit Betahistin. Bei therapierefraktären Beschwerden intratympanale Injektion von Kortison. Ultima Ratio operative Entlastung des Hydrops.
