Hidradenitis suppurativa (Acne inversa)

Definition:
Chronisch-rezidivierende, schmerzhafte Hauterkrankung, bevorzugt in intertriginösen Arealen, mit Abszess-, Fistel- und Narbenbildung. Die Ursache ist ungeklärt, im Hintergrund steht jedoch ein Verschluss der Haarfollikel mit nachfolgender Entzündung und Gewebezerstörung.
Häufigkeit:
Betrifft ca. 1 % der Bevölkerung, Männer sind häufiger betroffen als Frauen. Weltweites Vorkommen.
Symptome:
Schmerzhafte Knoten, die sich zu Abszessen entwickeln und rupturieren können; oft kommt es zur Sekundärinfektion des umgebenden Gewebes. Häufigste Lokalisation: Achselhöhlen und Leistengegend, aber auch Brust, Perineum sowie Perianalbereich. Mögliche schwere Verläufe mit Fistel- und Narbenbildung.
Befunde:
In den typischen Bereichen schmerzhafte, rote Knötchen, Abszesse sowie ggf. Fistelbildung oder strangartige Narben. Bei Männern häufiger perianales Auftreten.
Diagnostik:
Die Diagnose wird klinisch gestellt.
Therapie:
Die Behandlung erfolgt abhängig vom Schweregrad und Verlauf der Erkrankung. Möglich sind z. B. topische oder systemische Antibiotikabehandlungen, Behandlung mit Externa, chirurgische Interventionen oder Lasertherapie.
