Postthrombotisches Syndrom (PTS)

Definition:
Zusammenfassender Begriff für alle Folgezustände nach tiefer Bein- oder Beckenvenenthrombose. Durch persistierende Obstruktion und/oder Venenklappeninsuffizienz entsteht eine venöse Hypertonie.
Häufigkeit:
20–50 % der Patient*innen mit TVT entwickeln ein postthrombotisches Syndrom. Jährliche Inzidenz 1–3/1.000 Einw.
Symptome:
Typische Symptome sind Schmerzen, Krämpfe, Schwere- und Spannungsgefühl, Parästhesien, Juckreiz.
Befunde:
Typische Befunde sind Ödeme, Hautinduration, Hyperpigmentierung, Rötung, Wadenkompressionsschmerz, Ulzeration.
Diagnostik:
Klinische Diagnose, evtl. ergänzende Bildgebung (Duplexsonografie). 
Therapie:
Überwiegend konservative Behandlung mit Allgemeinmaßnahmen, physikalischer Therapie und Kompressionsbehandlung. Im Einzelfall sind bei schwerem PTS chirurgische oder endovaskuläre Therapien eine Option.
