Chronische Pankreatitis

Definition:
Chronisch-rezidivierende Entzündungsschübe mit Ersatz des Pankreasparenchyms durch fibrotisches Bindegewebe, dadurch  
fortschreitender Verlust der exokrinen und endokrinen Pankreasfunktion.
Häufigkeit:
Prävalenz 40–70/100.000, Verhältnis Männer zu Frauen 2:1. 70–80 % der Fälle durch Alkohol bedingt.
Symptome:
Leitsymptom sind intermittierende oder chronische Bauchschmerzen. Gewichtsverlust, Steatorrhö als Symptome der exokrinen Insuffizienz. Bei endokriner Insuffizienz Symptome eines Diabetes mellitus oder durch Hypoglykämien.
Befunde:
Zeichen einer Mangelernährung mit Gewichtsverlust, evtl. schmerzhafte abdominelle Palpation; ggf. Zeichen eines alkoholbedingten Leberschadens.
Diagnostik:
Fäkaler Elastase-Test zum Nachweis einer exokrinen Insuffizienz, Glukose/HbA1c zur Diagnostik einer endokrinen Insuffizienz. Abdomensonografie als primäres Bildgebungsverfahren, ergänzende Bildgebung zur definitiven Diagnosestellung (Endosonografie, MRT oder CT).
Therapie:
Lebensstiländerung mit Alkohol- und Nikotinkarenz. Medikamentöse analgetische Therapie. Bei exokriner Insuffizienz Pankreasenzymsubstitution. Behandlung eines Diabetes mellitus. Bei chronischem Schmerzsyndrom oder sonstigen Komplikationen (Cholestase, Pankreasgangobstruktion/-konkremente, Pseudozysten) endoskopische oder chirurgische Therapie.
