Benigner paroxysmaler Lagerungsschwindel

Definition:
Kurze Schwindelattacken (ca. 20 sec), ausgelöst durch Lagewechsel des Kopfes gegenüber der Schwerkraft, Übelkeit und Oszillopsien als mögliche Begleitsymptome. Typische Auslöser sind Hinlegen, Aufrichten oder Umdrehen im Bett, Bücken oder Kopfreklination.
Häufigkeit:
Bei ca. 20 % der Patient*innen mit Schwindel besteht ein benigner paroxysmaler Lagerungsschwindel.
Symptome:
Starker anfallsartiger Drehschwindel, evtl. vegetative Begleitsymptomatik (Übelkeit, Erbrechen).
Befunde:
Nystagmus, Schlagrichtung abhängig vom betroffenen Bogengang.
Diagnostik:
Die Diagnose beruht auf der typischen Anamnese und Auslösbarkeit durch Lagerungstests.
Therapie:
Behandlung durch verschiedene ärztlich durchgeführte Lagerungsmanöver und/oder Eigenübungen der Patient*innen. Hohe Erfolgsrate, allerdings auch häufig Rezidive, die erneut behandelt werden können.
