Muskellogensyndrom / Kompartmentsyndrom

Definition:
 Zustand erhöhten Drucks innerhalb einer Muskelloge, der die Durchblutung und Funktion des mit Faszien umschlossenen Gewebes kompromittiert.
Häufigkeit:
 Häufige Komplikation bei Frakturen des Unterschenkels.
Symptome:
Initial Vernichtungsschmerz durch Ischämie, später Parästhesien und Paresen.
Untersuchung:
Dehnungsschmerz der betroffenen Muskulatur, Sensibilitätsausfälle und motorische Ausfälle der betroffenen Nerven.
Diagnostik:
Kombination aus Klinik sowie intrakompartimenteller Druckmessung.
Therapie:
Schnellstmögliche Fasziotomie, bei chronischem Kompartment initial konservativer Therapieversuch durch Sportkarenz und Dehnen.
