Achillessehnen-Tendopathie

Definition:
Degenerativer Umbau der Achillessehne mit Schmerzen und Funktionseinschränkung.
Häufigkeit:
Inzidenz von 2–3 Fällen auf 1.000 Erwachsene, gehäuft bei Sportler*innen.
Symptome:
Dorsale Fersenschmerzen bei körperlicher Belastung.
Befunde:
Achillessehne palpatorisch verdickt und druckdolent, evtl. mit tastbaren Knötchen.
Diagnostik:
In der Regel klinische Diagnose möglich. Bei unklaren Befunden Sonografie und ggf. MRT. Bei Insertionstendinopathie laterale Röntgenaufnahme zum Ausschluss knöchernen Anomalien (Haglund-Exostose).
Therapie:
Exzentrische Übungen für die Wadenmuskulatur, ggf. ergänzt durch Fersenerhöhung. Bei persistierenden Beschwerden Stoßwellentherapie. Ultima Ratio Operation.
