Bipolare affektive Störungen

Definition:
Äußert sich in Form einer oder mehrerer manischer (Typ I) oder hypomanischer (Typ II) Episoden und einer oder mehrerer depressiver Episoden, die von symptomarmen oder -freien Phasen unterbrochen werden.
Häufigkeit:
Die Lebenszeitprävalenz wird auf etwa 1 % beziffert. Schließt man leichtere Formen mit ein, dann liegt sie bei etwa 5 %.
Symptome:
Manische Symptome sind u. a. gehobene, expansive oder gereizte Stimmung. Kennzeichen einer Depression sind gedrückte Stimmung, Interesse- und Antriebslosigkeit.
Befunde:
Manische Patient*innen sind oft sehr unruhig, mitunter auch gereizt-aggressiv, sodass sich ein normales Gespräch schwierig gestalten kann. In der depressiven Episode fallen evtl. Konzentrationsstörungen, psychomotorische Verlangsamung oder ängstliche Agitiertheit auf.
Diagnostik:
Von entscheidender Bedeutung sind neben der klassifikatorischen Diagnostik nach ICD-10 die dimensionale Diagnostik mithilfe verschiedener Selbst- und Fremdbeurteilungsinstrumente sowie die Differenzial- und Komorbiditätsdiagnostik.
Therapie:
Die Aufklärung und Therapieplanung erfolgt im Trialog zwischen Behandelnden, Patient*innen und, wenn diese einverstanden sind, den Angehörigen. In der Regel ist eine medikamentöse Akuttherapie und/oder Phasenprophylaxe angezeigt. Auch psychotherapeutisch-psychoedukative Elemente haben einen hohen Stellenwert. Zur ergänzenden Behandlung kommen ggf. Licht- oder Wachtherapie sowie kreative, entspannende und handlungsorientierte Verfahren zum Einsatz. Bei unzureichendem Ansprechen auf die medikamentöse Behandlung kann eine Elektrokonvulsionstherapie (EKT) erwogen werden.
