Pathologisches Glücksspiel

Definition:
Psychische Störung mit starkem Drang zum Glücksspiel und Vernachlässigung von anderen Dingen des täglichen Lebens. Es besteht eine signifikante Komorbidität zu anderen psychischen Störungen. Ein niedriger sozioökonomischer Status scheint prädisponierend zu sein.
Häufigkeit:
Die Anzahl pathologischer Spieler in Deutschland wird für 2017 auf ca. 180.000 geschätzt.
Symptome:
Zu den Symptomen zählen eine mangelnde Impulskontrolle in Bezug auf Glücksspiele sowie Verhaltensmuster, unter denen die Patientin/der Patient und ggf. auch das Umfeld leiden.
Befunde:
Das Spielverhalten kann zu Depressionen und u. U. einem erhöhten Suizidrisiko führen. Darauf ist in der Anamnese besonders zu achten.
Diagnostik:
Die Diagnose kann über einen Fragebogen gesichert werden.
Therapie:
Therapieziele sind neben einer Kontrolle und Reduktion der Spielfrequenz auch eine Begrenzung der Folgeschäden und die Rückfallprävention. Die Therapie ist multimodal und beinhaltet Psychotherapie, Selbsthilfegruppen und ggf. zusätzlich eine medikamentöse Behandlung (Off-Label-Use).
